EECP - Enhanced External CounterPulsation

A safe & effective treatment for angina and heart failure

EECP is a non-invasive out-patient treatment to help many people with angina and heart failure, including when the disabling symptoms of angina are uncontrolled or worsening despite maximal medical and surgical intervention.

Clinical Trials show that approximately 80% of angina patients experience significant symptom relief after EECP that may last up to three years.

What benefits occur after EECP?

Clinical Studies show that people notice:

Reduced frequency or complete elimination of angina symptoms.
Better ability to exercise free from chest pain and breathlessness.
Decrease in need for anti-angina nitrate medication

Clinical tests show:

Decreased exercise-induced signs of angina on ECG (prolonged time to ST depression on exercise stress testing)
Increased blood supply to heart muscle , demonstrated by myocardial perfusion scan techniques before and after EECP (Technetium scan, Cardiovascular MRI, stress ECHO)

How do people feel after EECP treatment?

Patients often report a greatly improved ability to participate in activities of daily living, such as the ability to walk further, carry heavier objects, enjoy gardening and leisure activities and be more active and independent without having angina and breathlessness.

We aim to set goals with our patients at the start of the treatment process, and find that EECP is most effective if the increase in exercise ability gained after treatment is maximised through continued regular exercise, activity and ongoing cardiac rehabilitation.

IS EECP SAFE?


  • EECP is an extremely safe and well tolerated treatment
  • Occasionally patients experience minor skin irritation due to the rubbing from the cuffs, but this is rarely a problem with padding and use of emollient cream between treatments
  • At our clinic, all patients are assessed by a Consultant Cardiologist to ensure their suitability for EECP
  • A fully qualified senior nurse is present throughout treatment with a supervisory doctor on site

Is EECP suitable for my condition?

You may be a candidate for EECP if you:

  • Have chronic stable angina or diagnosed heart failure.
  • Are not receiving adequate relief from angina by taking medication.
  • Do not qualify as a candidate for invasive procedures.
  • Have exhausted invasive treatments without lasting relief of symptoms.
  • Are unwilling to undergo surgery or angioplasty.
  • Want to explore alternatives to bypass surgery or angioplasty.


Does EECP offer new hope?

For patients who have undergone multiple invasive procedures and for whom addition surgery carries excessive risk, EECP may be the only way to obtain relief form severe angina or heart failure symptoms.

There is an increasing trend internationally for patients who have significant risk factors such as obesity, strong family history of heart disease, high cholesterol, diabetes, smoking and high blood pressure to undergo self-funded EECP treatment on a prophylactic basis  (i.e. before the onset of angina or heart problems).

What does it feel like to have EECP?

  • The cuffs around the legs inflate and deflate in time with your heart beat. The cuff sensation is similar to having your blood pressure taken by your doctor or practice nurse, similar to a firm massage but should not be painful.
  • Many people listen to music or audiotapes, others just relax or enjoy talking to our fully qualified nurses who stay with our patients throughout the course of the treatment.
  • If you wish to speak to a patient who has had EECP therapy then this can be arranged by the clinic. All our patients have the opportunity to see the EECP bed before decisions regarding treatment.
  • The treatment is carried out in our comfortable and well-equipped clinics with one-to one nursing support throughout and  supervisory doctors on the clinic site.
  • We ensure our clinics have the highest level of privacy, courtesy and confidentiality.

How should I prepare for EECP treatment?

We recommend that for your comfort that you do not have a large meal or lots of caffeine and fluids within two hours of starting treatment.

We can easily interrupt treatment for a toilet break but it is best if possible to have a continuous treatment session of one hour.

We will provide you with a pair of tight thin trousers to minimise any rubbing from the cuffs and recommend that you moisturise your legs between treatments. You may wish to put these on at home under your clothing or use our private changing area.

Step by step guide to the treatment


  1. During treatment three ECG stickers are placed on the chest to measure the heart’s contraction and relaxation (systole and diastole).
  2. The patient rests on a bed and has blood pressure cuffs placed around the calf, thigh and buttock area over thin clothing.
  3. At the exact moment of diastole or heart relaxation, the blood pressure cuffs inflate starting from the calf sequentially up to the buttocks which causes a counter pulsation of blood.
  4. The cuffs then rapidly relax and deflate in time for the hearts contraction phase. This feels like a firm massage in time with your heart beat.
  5. Unlike bypass surgery or angioplasty there is no need for any overnight stay in hospital. The patient attends for a total of 35 hours treatment on the EECP bed. This is usually completed in one hour treatment sessions per day, but can be increased to two hours per day with a short break between the first and second hour.
  6. On this regime, the treatment cycle can be completed in four weeks - otherwise the treatment duration is seven weeks.
